How we store it: Your data is stored securely within our ministry's CRM platform. We retain it for the duration of your group participation and reasonable follow-up period.

This is an interest form, not a binding enrollment. Submitting this form places you on a waitlist for a Journey Tools small group. Placement is not guaranteed and is subject to group availability and facilitator confirmation.

Participation is voluntary. You may withdraw from the waitlist or from the program at any time by contacting your facilitator or Cross Care Ministries directly.

Communications. By submitting this form, you agree to receive group-related communications (email and/or text) from Cross Care Ministries or your assigned facilitator regarding your Journey Tools group.

Program content. Journey Tools groups address topics including personal growth, relationships, and emotional wellness. Participation does not constitute clinical counseling or therapy. The purpose of the group is only to discuss the educational content of the videos and workbook. For mental health emergencies, please contact a licensed professional or call 988.

Accuracy. You agree that the information you provide is accurate and belongs to you.
